Description

เพิ่ม column วันที่ตรวจฟิล์ม

เพิ่ม 2 dolumn (เพิ่มเติมจาก program spec)

-โรงเรียน >> แสดงชื่อโรงเรียนจากส่วนหัวในงาน ACD
-indexcase >> แสดงชื่อ indexcase

แก้ไขการแสดงผลใน column "สถานที่ตรวจบำบัด" "รูปแบบ" และ "ประเภท" ตามรูปที่แนบมาค่ะ

-ตอนเพิ่มข้อมูลแบบกลุ่ม ระบุข้อมูลโรงเรียน และ indexcase ไป แต่ในหน้านี้ไม่แสดงค่ะ
